Completed forms may be dropped off at the front desk of the museum or mailed to:

For Students:

Student Permission Forms are due along with a copy of your student roster. Please highlight or circle the names of any students on your roster who have not been given permission to participate. 

For Community Members, Volunteers, and Subjects in Artwork:

During in-school visits, expeditionary field trips, and as part of their final art projects, students often interview and photograph community members, volunteers, and other subjects.  Please have all individuals impacted by this program complete this photo release form and submit it to the Arts in Education team.
